CArE accepts referrals for individuals living with arthritis and related musculoskeletal conditions who may benefit from assessment, education, treatment, and self-management support. Our goal is to provide timely, patient-centred care through an interprofessional team approach.
Referrals help ensure patients are connected with the most appropriate services and programs based on their individual needs. Depending on the referral, patients may be scheduled for assessment, education sessions, exercise programs, or individualized treatment support.
Currently we are offering a self-referral op9on to our Osteoporosis Group Education Workshop. The CArE Osteoporosis Self-Referral Process allows patients to express their interest in an Osteoporosis Workshop without a healthcare provider referral. Patients complete the online Self-Referral Form and email it to [email protected]. A CArE team member will contact the patient with workshop information and, if appropriate, register them for an available session. Participants receive practical education and resources to support bone health, safe movement, fall prevention, and long-term osteoporosis management.
