Building the Future of CArE

CArE’s 2026–2031 Strategic Plan sets our direction for the next five years and builds on our commitment to delivering exceptional, patient-centred arthritis care.

Developed with input from patients, staff, clinicians, partners, and our Board, the plan focuses on strengthening our organization, empowering the people we serve, improving access to care, building meaningful partnerships, and demonstrating the impact of our work.

Our five strategic priorities will guide how CArE continues to grow, innovate, and improve arthritis care in the years ahead.

1. Build Our Capacity

Strengthening our people, systems, and resources to deliver exceptional care.

We will invest in our people, improve our processes and digital capabilities, and strengthen our infrastructure and financial sustainability so CArE can continue to provide high-quality care now and into the future.

2. Empower Our Clients

Equipping people to take charge of their health and thrive.

We will provide personalized care, education, tools, and resources that build knowledge, confidence, and self-management skills while ensuring that care reflects the individual needs and lived experiences of the people we serve.

3. Partner for Outcomes

Working together to deliver better care and stronger results.

We will strengthen partnerships with patients, caregivers, healthcare providers, community organizations, and other partners to improve care pathways, make better use of available resources, and help people access the right care at the right time.

4. Expand Our Reach

Bringing CArE to more people, in more places, in more ways.

We will explore new and flexible ways to deliver care, expand access for underserved communities and new patient populations, and build the foundation for CArE’s model of arthritis care to reach more people.

5. Demonstrate Our Impact

Showing how CArE makes a measurable difference.

We will strengthen how we measure and evaluate outcomes, using research, data, and digital tools to demonstrate improvements in health, patient experience, and health system performance. We will use what we learn to continuously improve our services and share evidence of what works.
